| 3. | Complete legal forms, nominating your local doctor, lawyer or other professional adviser to receive the reports. |
| 4. | Ensure Family Law Act Form 2 (in the document pack) is completed and signed by a Justice of the Peace or solicitor. |
| 5. | Take a passport size photo and completed legal form for each donor to the collection appointment. |
